Retirement Announcement
After 31 years as a family doctor, 17 of which have been here in this South Pasadena office, I have decided that it is time for me to start the next chapter of my life. I will be retiring and closing my medical practice effective December 17th, 2021.

Thank you to all of my patients, past and present, for the honor and privilege of serving as your family doctor and entrusting me with your medical care over the years. I have enjoyed getting to know each and every one of you, some of you since birth. I appreciate you letting me into your lives and sharing your stories with me.
If you need your medical records transferred, please download a Medical Record Request Form and send it to me by e-mail, fax or regular postal mail.
There is no charge to transfer your medical records to another physician. If you request that your medical records be released to yourself, I will charge 25 cents per page for medical records printed to paper. There is no charge for digital copies of your medical records sent to yourself via e-mail, secure messaging, or fax.
My website will stay up for at least one year after I retire, so there is no need to rush to request your medical records. I recommend that you establish care with another doctor first, and then request that your medical records be sent if your new doctor wants them. Alternatively, you may request that your medical records be sent directly to you.
